HEALTH CHECK-UP SCHEME FOR CHILDREN AND ADULT

HISTORY and PHYSICAL EXAMINATION

		Complete Hemogram (Hb., Hct., RBC, WBC, DLC & Platelets)
		Urinalysis (Routine & Microscopic)
		X-Ray Chest, PA & Lateral Views
		Mantoux Skin Test

1. Ten (10) percent discount will be given to each student up to the age of eighteen years if he / she forms a part of a group of twenty students at any given time. Discount will be offered to the school in the form of coupons, which will remain valid up to one year from the date of issue. Such discount coupons may be utilized by any student / staff member towards treatment at Indus Hospital at the written recommendation of the school principal / headmaster.

2. Working days: Mondays thru Fridays 9.00 a.m. to 5.00 p.m., Saturdays 9.00 a.m. to 4.00 p.m.

3. Holidays January 25 & 26, April 15, August 15 and October 02.

1. In addition to the above, certain other tests i.e. Ultrasound, C.T. Scan, T.M.T., P.F.Ts., Lipid Profile, etc., may be indicated in high risk patients, depending upon the history of the patient and judgment of the physician. If required, they will be charged extra. Special tests not included in the abridged rate list will be got done from reputable national / international medical laboratories in India or USA

2. Discount of five (5) percent shall be given to each patient in a group of five (5) at a time.

3. Working days: Mondays thru Fridays 9.00 a.m. to 5.00 p.m., Saturdays 9.00 a.m. to 4.00 p.m..
