| DISCHARGE
POLICIES
1. Your physician knows when you are ready to be
discharged from the Hospital. While the Hospital endeavors
to provide speedy and cost effective health care in
the most efficient manner it knows how to, any pressure
tactics, employed by the patient/relatives for premature
discharge, are strongly resented by the physician
in-charge since he may be held liable for Violation
of Discharge guidelines by the Hospital. Premature
discharge is tantamount to non-compliance on the part
of the patient which is detrimental to a healthy doctor-patient
relationship.
2. A premature discharge without the consent of
the Physician in-charge may increase the liability
of the physician in-charge and Indus Hospital. Patients
leaving the Hospital against medical advice are required
to sign certain documents (L.A.M.A. Forms). All such
patients shall forfeit their rights to obtain copies
of their medical records. They may not be welcome
as patients at Indus again.
3. Normal check-out time is 1:00 P.M. All discharge
orders must be written before 11:00 A.M. on the day
of the discharge.
4. Hospital discharges are not made on Sundays and
Holidays, unless prior arrangements are made with
the Business Office and the Physician in-charge during
the normal working hours.
5. Prior to discharge, your physician will advise
you regarding the activity, diet, medications, precautions,
if any, and subsequent follow-up visits to the Hospital.
Please follow the instructions carefully. After the
Hospital discharge, you may contact your Physician
by telephone, fax or e-mail for clarifications, if
any. Please do not forget to bring your medical records
during follow-up visits to the Hospital.
6. Ordinarily, your in-door Hospital Records and
X-ray/Ultrasound films remain in the custody of Indus
Hospital for a period of 10 years. X-ray/Ultrasound
films may be borrowed for review by qualified consultants
elsewhere after the Hospital discharge, provided the
Hospital Departmental Procedures for doing so are
strictly followed and all the films are returned in
the same condition within a period of thirty (30)
days. Failure to do so on the part of the patient
will absolve Indus Hospital of any subsequent medico-legal
liability.
7. At the time of Hospital discharge, you will receive
a copy of Patient Feedback Form which you may complete
at your own convenience at home. Do not forget to
send it back as your opinions and suggestions are
valued highly by the Quality Assurance Department
of the Hospital.
8. Hospital property at the time of discharge must
be handed over to the Nurse in-charge.
9. Patients in need of transportation at the time
of discharge may contact the Reception Desk for assistance,
atleast, one day in advance. Subject to availability
, Hospital vehicle may be hired for a distance not
to exceed twenty-five (25) Kms. One way, provided
, the journey involves paved roads only.
10. In the event of a death, Release of Dead Body
Forms must be completed before the body can be handed
over to the relatives.
