Normal
check-out time is 1:00 P.M. All discharge orders must be written
before 11:00 A.M. on the day of the discharge.

Hospital
discharges are not made on Sundays and Holidays, unless prior
arrangements are made with the Business Office and the Physician
in-charge during the normal working hours.

Prior
to discharge, your physician will advise you regarding the
activity, diet, medications, precautions, if any, and subsequent
follow-up visits to the Hospital. Please follow the instructions
carefully. After the Hospital discharge, you may contact your
Physician by telephone, fax or e-mail for clarifications,
if any. Please do not forget to bring your medical records
during follow-up visits to the Hospital.

Ordinarily,
your in-door Hospital Records and X-ray/Ultrasound films remain
in the custody of Indus Hospital for a period of 10 years.
X-ray/Ultrasound films may be borrowed for review by qualified
consultants elsewhere after the Hospital discharge, provided
the Hospital Departmental Procedures for doing so are strictly
followed and all the films are returned in the same condition
within a period of thirty (30) days. Failure to do so on the
part of the patient will absolve Indus Hospital of any subsequent
medico-legal liability.

At
the time of Hospital discharge, you will receive a copy of
Patient Feedback Form which you may complete at your own convenience
at home. Do not forget to send them back as your opinions
and suggestions are valued highly by the Quality Assurance
Department of the Hospital.

Hospital
property at the time of discharge must be handed over to the
Nurse in-charge.

Patients
in need of transportation at the time of discharge may contact
the Reception Desk for assistance, atleast, one day in advance.
Subject to availability , Hospital vehicle may be hired for
a distance not to exceed twenty-five (25) Kms. One way, provided
, the journey involves paved roads only.

In
the event of a death, Release of Dead Body Forms must be completed
before the body can be handed over to the relatives .
